About attend international conference, please apply to support early two months.

The conference support apply to have two kind.

(1)Please sign the Conferences Application Form.

(2)Print the Conferences Application Form and required documents.

  (Please see the second page of the provisions.)

(3)You must signatures on the form.

Please your supervisor signatures on the third Page of Conferences Application Form .

(4)Dateline is the conference early two months.

2.成杏基金會(NCKU Hospital):

(1)Please give me three information.

  A. Title of paper to be presented

  B. Method of paper to be presented or mission(Oral or Poster)

  C. Amount requested：Airline ticket NT______、Daily living allowance NT_____  

                     Registration fee NT_____   

  I have more information that I will give you the Application Form.

(2) Required documents have 1. Conference introduction.

2. Photocopy of paper acceptance letter.

3. Photocopy of abstract and full paper (except Chinese) to be presented.

(3)All the documents must print, and the Application Form must your supervisor signature.

(4) Dateline is the conference early one week.
